How to submit an insect sample

  1. Put insect in small jar or container with tight fitting lid.
  2. Pour about one tablespoon of rubbing alcohol into container.
  3. Securely tighten jar lid or container top.
  4. Fill out the appropriate diagnostic sheet as completely as possible. Click on the following links to download the appropriate form. (You must have the Adobe Reader program installed on your computer. You can download the free program by clicking on the link at the bottom of this page)
  5. Put the sample and diagnostic form in padded envelop or sturdy box, so insect and container are not processed through postal machinery.
  6. Mark the package with your name, mailing address, and phone number.
  7. Mail package to:
