How to submit a plant disease sample

  1. Select plant disease samples with various stages of the disease symptoms and also healthy parts.
  2. Place fresh samples in dry paper towel or newspaper or brown paper. Do not add water. Submit the samples as soon as possible after you collect them. Do not enclose in plastic bag. Package samples individually.
  3. Fill out the appropriate diagnostic sheet as completely as possible. Click on the following links to download the appropriate form. (You must have the Adobe Reader program installed on your computer. You can download the free program by clicking on the link at the bottom of this page)
  4. Put the sample and diagnostic form in a padded envelope or sturdy box.
  5. Mark the package with your name, mailing address, and phone number.
  6. Mail packages to arrive on weekdays to the following address:
